Instructions for Department Logic Model
Logic: Given the impact you desire for students who interact with your department, how do you tie
the resources you need and the activities you produce…
with the proper documentation to verify that…
you have succeeded in producing measurable outcomes
that lead to the desired long-term student impact?
Please complete the Logic Model sheet, pasting each impact statement into a separate box and the
resources and activities from the Change Theory worksheet which you believe are associated with
that impact. Then complete the Output (telling me what documentation you will use such as counts,
surveys, etc.) and Outcome (the measurable way in which the student will benefit as a result of
your services) portions. Do not attempt to reach perfection; brainstorm first and refine later. Use
concise, simple words and try to avoid “education speak” - act as if you are someone attempting to
explain to a student, parent or stranger what you are about and how you hope to affect the student’s
future. Feel free to expand the boxes, but keep each statement concise. For each box, limit your
sentences to the core of your values, do not go into detail, but think holistically about your services.
