POLICY:
● Diagnostic radiologic procedures shall be performed upon the written request of a member
of the medical staff, a member of the house staff, those physicians and dentists who are
not members of the medical staff but who have authorization to request such support
services, authorized podiatrists and, to the extent permitted by law, other persons
authorized by the hospital and licensed to engage in direct treatment of patients.
● All requests for radiologic procedures shall contain the reasons for the examination. A
space is provided on the x-ray request for this information. It is the responsibility of the
person filling out the request to provide this information and to also initial said request.
Outpatient requests shall be processed by the Imaging Services Department personnel.
