Grampanchayat i.e. Form No.9.
b) True Copy of the cast – Certificate in case of admission is sought on
the basis of reserved category.
c) Affidavit in case the child’s name is not included in the Birth
certificate.
d) School Leaving Certificate of the previous school, if any.
e) Mark list of the last standard passed.
f) Three identity card size photographs of the child.

2) Admission can be cancelled after the first unit test if the student is found
unit for an English Medium School.
