This is a legal document in which you, the parent, designate a person to take care of your child(ren) if you ever become incapacitated. Married couples have the assurance that if anything happened to either one of them, their spouse could step in and take care of the children. However, if you are single, separated, or married to someone who is not your child’s parent you should make special provisions in case of your incapacity. This document is designed to be used in an emergency and it springs into action once the conditions of your incapacity are met. This form gives the guardian broad authority to act as parent.
